Annual Membership Program

Due to the ever-changing world of healthcare, and physical therapy in particular, Preferred Physical Therapy Associates has established an Annual Membership Program to maintain the high quality of care we have provided for nearly two decades. Many of our services, such as Cold Laser Therapy, Shockwave Therapy, and Cupping Therapy, are not reimbursed by insurance. This program allows us to continue offering these enhanced services while building lasting relationships and trust with our patients.

All patients of Preferred Physical Therapy Associates are eligible to join the Annual Membership Program for a small fee. While membership is optional, we encourage all patients to join to take full advantage of the benefits for both Physical Therapy (PT) and Occupational Therapy (OT).

Benefits of the Preferred Membership Program:

The following services are included at no additional charge for members (these services are not reimbursed by insurance):

  • Cold Laser Therapy – $30 value per treatment
  • Shockwave Therapy – $30 value per treatment
  • Cupping Therapy – $30 value per treatment
  • Paraffin Therapy – $30 value per treatment
  • Custom Orthotics – $50 discount applied
  • Electrodes for Electrical Stimulation – $5 value per treatment
  • Home Exercise Elastic Band – $15 value per disbursement
  • Kinesio-Taping – $30 value per application
  • Supplies related to TMJ treatment – $20 value
  • NSAID Topical Gel with/without Ultrasound Gel for phonophoresis – $15 value
  • Access to PT Wired Exercise App with 2 exercise updates – $30 value
  • Use and analysis of Vertigo Goggles in all tested positions – $75 value per treatment
  • Complimentary 30-minute session with a therapist when signing up for 3 hour private pay sessions – $90 value
  • Complimentary Membership Gift at the completion of your initial plan of care

Membership benefits are available for both PT and OT services, so you can receive the same high-quality care across disciplines.

Membership Details

  • Annual Membership Fee: $109 (covers all services above for one year)
  • Non-members may still receive the above services, but will be charged individually per treatment item (one item charge per treatment).
